Job summary

St. David's HealthCare is seeking a Registered Nurse Case Manager to support patients and families through their care journey. You will coordinate services, develop care plans, and partner with physicians and the care team to ensure safe transitions and high-quality care.

You will guide education for patients and families, document assessments, and collaborate with leadership to promote compassionate, excellent care. The role emphasizes adherence to the organization’s values and standards.

Qualifications

• RN credentials and a Bachelor’s degree required.
• Experience coordinating care plans or case management is preferred.
• Ability to collaborate with physicians, nurses, and leaders to ensure safe transitions.

Responsibilities

• Guide patients and families through program orientation, explain rehab philosophy and patient rights.
• Coordinate education for patients and families to support participation in treatment planning.
• Develop and implement individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and discharge planning evaluations with clear documentation.
