Job summary

Research Medical Center, part of HCA Healthcare, seeks an RN Case Manager to support patients and families through their care journey. You will coordinate services, develop care plans, and partner with physicians, nurses, and leaders to ensure safe transitions and clear communication.

You will guide patients through program orientation, explain insurance benefits, participate in discharge planning, and document findings to support a collaborative, patient-centered approach across the care

Qualifications

• RN and RN license required.
• Experience in case management or discharge planning preferred.
• Ability to coordinate care across multiple disciplines.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths and needs.
• Complete psychosocial assessments and discharge planning evaluations with clear documentation.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.
• Promote values of compassion, respect, and excellence in interactions.
