Job summary

HonorHealth in Phoenix, AZ seeks an experienced RN Case Manager for day shifts to guide care coordination for high-risk acute care patients and manage discharge planning. You will evaluate admission necessity, coordinate resources, and liaise with the care team to ensure safe transitions.

We prefer candidates with hospital case management background, RN licensure, and an understanding of regulatory and payer guidelines to advocate for patients and families throughout the care continuum.

Qualifications

• 3 years of clinical hospital experience required.
• 5 years as an RN in Case Management preferred.
• 7 years as a Registered Nurse (RN) preferred.
• Experience in outpatient clinical settings with PCP, Home Health, or Rehab preferred.
• RN license required; Certified Case Manager preferred.

Responsibilities

• Guides care coordination for high-risk acute care patients and facilitates discharge planning.
• Analyzes medical records to determine admission necessity, treatment, and length of stay per patient needs and guidelines.
• Advocates for patients and families and liaises with the care team to ensure needs are addressed.
• Collaborates with physicians and assists in facilitating healthcare resources for timely service delivery.
• RN Case Manager coordinates with patients, providers, and stakeholders to ensure safe discharge.
