Job summary

Saint Francis Hospital, Inc. is seeking a Case Manager (RN) for the weekend shift (Fri, Sat, Sun 7am–7pm) to coordinate discharge planning and ensure safe transitions of care.

The role works with patients, families and the care team to minimize readmissions and optimize care pathways. The position requires an RN license (Oklahoma or multi-state), typically 3+ years of clinical and discharge planning experience, and strong communication skills.

Qualifications

• Completion of basic nursing curricula as approved by state board of nursing.
• Holds or is entitled to hold a nursing diploma or degree.
• Active RN license (multi-state or Oklahoma).
• CCM certification preferred but not required.

Responsibilities

• Plan patient care with physicians and healthcare team.
• Ensure discharge plan aligns with clinical course and ongoing needs.
• Identify potential discharge barriers and address them proactively.
• Participate in multidisciplinary patient rounds.
• Coordinate discharge planning with patient, caregivers, and health team.
• Evaluate progress toward clinical/functional goals.
• Initiate and coordinate discharge from acute care settings.
• Obtain payer authorization for post-acute services.
• Arrange care in home health, rehab, or SNF as needed.
• Assist in hospital-to-hospital transfers.
• Track avoidable days and report trends to leadership.
