Job summary

Saint Peter's Healthcare System is seeking a Case Manager RN to join our magnet-designated Care Coordination team. You will perform discharge planning evaluations, assess patient needs, and collaborate with interdisciplinary rounds to optimize care and reduce readmissions.

Requirements include a NJ RN license, BSN preferred or obtained within 3 years, 3–5 years in acute care, strong critical thinking and communication, and weekend/holiday availability.

Qualifications

• BSN preferred or must obtain within three (3) years of hire date.
• Licensed as a Registered Nurse in the State of New Jersey.
• Minimum of three (3) to five (5) years of experience in acute care nursing.
• Good critical thinking and communication skills.
• Must be willing to work on some weekend days and some holiday days.

Responsibilities

• Perform Discharge Planning evaluations including risk assessments for defined population within expected time frame.
• Provide Assessment and ongoing reassessment of a patient's diagnosis, prognosis, and anticipated care needs and goals.
• Collaborate with other disciplines; actively participate in interdisciplinary rounds.
• Collaboration includes progression of care, appropriate length of stay, integration of risk factors to reduce readmission, and integration for the care plan.
• Collaborate with the social worker daily to identify patients in need of social work intervention or with potential post-hospital discharge needs.
• Intervene and educate patients and families regarding anticipated complex home care needs including providers which are applicable to the patient’s goals and treatment preferences.
