Job summary

St. Christopher's Hospital for Children, part of Tower Health, is seeking an RN Case Manager to coordinate care for an assigned patient population. The role focuses on quality, resource utilization, and timely discharge planning in collaboration with the health care team.

Responsibilities include caseload management, effective care coordination, and advocacy for patient needs, with HIPAA compliance and patient-centered care guiding all activities.

Qualifications

• Bachelor's Degree required; 4-year degree or combination of relevant education & experience may be considered.
• PA Registered Nurse License is required.
• Relevant experience in case management or related field preferred.

Responsibilities

• Coordinate care for assigned unit-based patient population and promote quality of care.
• Oversee caseload management and follow-up to address complex patient needs.
• Plan for safe and timely discharge and ensure appropriate level of care.
