Job summary

Inova Fairfax Hospital in Falls Church, VA, seeks an Experienced Registered Nurse Case Manager 1 to join the Case Management Team. This is a full-time role, Monday through Friday with rotating weekends, focusing on discharge planning and care coordination across acute and post-acute settings.

The RN Case Manager will act as a liaison among patients, families and the healthcare team, ensure medical necessity and payer authorization, and participate in performance improvement activities.

Qualifications

• Certification: Basic Life Support (American Heart Association).
• Licensure: Licensed or eligible for licensure as an RN in Virginia or multi-state license.
• Experience: 1 year of case management and/or clinical care.
• Education: BSN or ADN with plan to obtain BSN within 5 years.
• Inpatient case management and discharge planning experience preferred.

Responsibilities

• Provide discharge planning and continuity of care for assigned patients.
• Coordinate referrals to clinics, home health, hospice and post-acute services.
• Collaborate with interdisciplinary team to assess discharge needs and care plans.
• Communicate status and goals to patients, families and care team.
• Monitor payer criteria and assist with approvals as needed.
