Job summary

Inova Loudoun Hospital is seeking anExperienced Registered Nurse Case Manager I to join the Case Management Team. This full-time role features Monday–Friday hours with rotating weekends and focuses on discharge planning and coordination of care for patients in acute and post-acute settings.

The RN Case Manager provides liaison between patients, families and the healthcare team, ensuring medical necessity and timely payer authorization while contributing to performance improvement initiatives.

Qualifications

• RN Case Manager 1 role requires discharge planning and coordination of care.
• Experience with utilization management and payer authorization.
• 1 year of case management or clinical care experience preferred.

Responsibilities

• Discharge planning and continuity of care for acute and post-acute patients.
• Coordinate referrals to clinics, home health, hospice, SNF, rehab and medical equipment.
• Collaborate with interdisciplinary team and families to manage discharge plans and ongoing care.
• Monitor and report on care plans, LOS, readmission indicators and resource use.

Education

Bachelor's Degree in Nursing or Associate's Degree in Nursing
