Job summary

Inova Fairfax Hospital, part of Inova Health System, is seeking an Experienced Registered Nurse Case Manager 1 to join the Case Management Team in a PRN capacity. The role emphasizes discharge planning and continuity of care for patients across acute and post‑acute settings.

You will coordinate services, collaborate with the interdisciplinary team, and facilitate referrals to clinics, home health, hospice and other post‑acute providers.

Qualifications

• Active RN license in Virginia
• 1 year of case management and/or clinical care experience
• Bachelor's Degree in Nursing or Associate's Degree with BSN plan within 5 years of start date

Responsibilities

• Coordinate discharge planning for patients in acute and post-acute settings
• Act as liaison between patients, families and healthcare team
• Identify and address barriers to care progression
• Collaborate with care team to coordinate referrals to clinics, home health, hospice, SNF and rehab facilities
• Utilize utilization management to determine medical necessity and efficiency of services
