Job summary

HCA Florida Orange Park Hospital is seeking an RN Case Manager to support patients and families through care journeys. You will coordinate services, connect resources, and develop individualized care plans in collaboration with physicians, nurses, and department leaders.

You will help ensure safe transitions, clear communication, and high-quality care across the continuum, aligning with HCA Healthcare values of compassion and excellence. This is an on-site role in Orange Park, FL.

Qualifications

• RN license required or diploma depending on state rules.
• Strong communication and collaborative skills.
• Ability to coordinate care across disciplines.

Responsibilities

• Coordinate services and develop care plans reflecting patient needs.
• Partner with physicians, nurses and leaders to ensure safe transitions.
• Explain rehab philosophy, Medicare benefits, discharge criteria and patient rights.
• Educate patients, families and caregivers to engage in treatment planning.
• Document psychosocial assessments and coordinate with care team.
• Promote values of compassion, respect and excellence in every interaction.
