Job summary

HCA Healthcare Summerville Hospital is seeking a part-time RN Case Manager to support patients and families through the care journey. You will coordinate plans of care, identify transition risks, and advocate for safe, quality outcomes across the continuum of care.

The role emphasizes care coordination, psychosocial assessment, and communication with physicians, patients, and social services to ensure appropriate post-acute care and resources.

Qualifications

• Requires graduation from an accredited school of nursing (ADN or higher).
• RN license in the practicing state or active compact/multi-state license.
• 1–3 years of clinical hospital nursing experience.
• 2 years in case management are highly preferred.
• Certification in Case Management or Utilization Review preferred.
• InterQual experience preferred.

Responsibilities

• Coordinate the plan of care for the patient stay and develop discharge plans.
• Identify patients at risk during transitions and ensure safe progression of care.
• Conduct psychosocial and medical assessments and reassess as needed.
• Ensure post-acute medical needs and levels of care align with Social Services.
• Use InterQual and approved criteria to evaluate care progression.
• Refer patients to third-party payers and disease/case management programs.
• Maintain professional communication with physicians, patients, families, and agencies.
