Job summary

Amergis is seeking an RN Case Manager to coordinate continuum of care for assigned patients, ensuring optimal use of resources and adherence to medical regimes. The role supports care plans across facilities and requires compliance with applicable standards.

Qualifications

• At least one year of Case Management experience preferred
• Complies with all relevant professional standards of practice
• Current CPR if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
