Job summary

Molina Healthcare is seeking an RN Case Manager to join our team and support Medicare members across the Illinois service area. This telephonic role may require occasional face‑to‑face meetings and occasional local travel.

The candidate should have prior Medicare/mCO experience and strong MS Office proficiency. Responsibilities include conducting assessments, developing care plans, coordinating with the care team, and monitoring progress.

Qualifications

• RN license active and unrestricted in state of practice.
• At least 2 years in health care, care management preferred.
• Understanding of EMR and HIPAA.
• Proficient with MS Office suite.

Responsibilities

• Complete comprehensive member assessments to determine care management eligibility.
• Coordinate care plans with members, caregivers, physicians and health professionals.
• Conduct telephonic, face‑to‑face, or home visits as required.
• Monitor and document progress toward goals and adjust care plans as needed.
• Maintain ongoing member caseload and outreach for care management.
