Job summary

UCI Health is seeking a skilled Case Manager to perform high-level assessments and create individualized care plans for an identified patient group. You will work within an interdepartmental team to ensure high-quality, cost-effective care and coordinated discharge planning for improved outcomes.

Requirements include a BSN, current RN/LSW/LCSW credentials or eligibility, and strong English communication. This on-site role supports UCI Health’s patient care mission and requires collaboration

Qualifications

• Must have 5+ years of clinical experience and 1 year of Case Management experience or a BSN.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
• Excellent written and verbal communication skills in English.
• Current Licensed Registered Nurse, Social Worker (MSW or LCSW), or Licensed Healthcare Therapist eligible for independent assessments.
• Computer literacy and proficiency.
• All external candidates must have a BSN.
• Ability to manage multiple concurrent projects and maintain a work pace.
• Ability to facilitate and lead interdisciplinary rounds.
• Ability to establish and maintain effective working relationships across the Health System.

Responsibilities

• Perform high level assessment and care planning for an identified group of patients which results in self-care outcomes upon discharge.
• Collaborate with an interdepartmental team to ensure delivery of high quality, cost-effective patient care.
• Coordinate discharge planning and post-discharge support to optimize patient outcomes.
