Qualifications

• Bachelor’s degree in nursing.
• Minimum of 3 years of clinical experience.
• Excellent verbal communication and written documentation skills.
• Excellent customer service and interpersonal skills across all organizational levels.
• Strong problem-solving, analytical, and decision-making abilities.
• Strong computer skills and knowledge.
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care preferred.
• Knowledge of preventive service guidelines, clinical practice guidelines, Medicare/Medicaid regulations, and case management principles.

Responsibilities

• Creates plan for care across the continuum, integrating patient/family preferences and values.
• Monitors patient medical necessity and level of care through assessments and records.
• Advocates for resources and removal of barriers.
• Maintains dialog with supervisor and care transition team for effective implementation and reevaluation of health plan.
• Acts as a resource for medical record documentation, level of care recommendations, and services related to diagnoses and post-discharge options.
