Qualifications

Responsibilities

• Identify, assess, plan, facilitate, care coordination, evaluation and advocacy for services to meet health care needs.
• Promote patient safety, quality of care and cost-effective outcomes.
• Provide effective discharge planning services for safe transitions across care settings.
• Collaborate with multidisciplinary teams and internal/external stakeholders.
