Henry Ford Health is seeking a Case Manager to join the hospital-based care team in Michigan. The role focuses on identification, assessment, planning, facilitation, care coordination, evaluation and advocacy to meet patient and family health care needs.

The Case Manager promotes patient safety, quality of care and cost-effective outcomes, and provides discharge planning to ensure safe transitions of care for patients with complex needs across the care continuum.

Qualifications

• Bachelor’s degree in nursing or Master’s degree in Social Work required.
• Minimum 3 years of clinical experience.
• Excellent verbal and written communication skills.
• Excellent customer service and interpersonal skills across all levels.
• Strong problem-solving, analytical and decision-making abilities.
• Strong computer skills.
