Job summary

Mass General Brigham is seeking a dedicated Case Manager RN for the Case Management Department in Boston. You will coordinate patient care across the continuum, assess needs, and facilitate services to optimize outcomes.

The role emphasizes collaboration with families, providers, and payers, ensuring efficient care transitions and adherence to institutional standards. Onsite, 30 hours weekly with a weekend monthly commitment.

Qualifications

• RN license required or RN license eligible
• 2-3 years case management, utilization review, or discharge planning experience preferred
• MA RN License required; BLS Certification preferred

Responsibilities

• Client assessments and planning.
• Modify patient treatment plans based on responses and conditions.
• Prepare patient/family for discharge.
• Communicate with third-party payers to obtain authorization for reimbursement of services.
• Review cases with medical directors as needed.
• Define care goals and provide education to patients and families.
• Maintain accurate, detailed reports and records.
