Qualifications

• RN Case Manager experience required.
• Must be a licensed RN in Massachusetts.
• Minimum of 2 years Med/Surg experience required.
• Case Manager certificate preferred.

Responsibilities

• Collaborate to enhance delivery of patient care services along the continuum of care.
• Evaluate, predict, and facilitate the trajectory of patient care.
• Coordinate assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
• Support by leaders, SMEs, and resource specialists to ensure high-quality care.
