Job summary

Qualifications

• Licensed RN in Massachusetts.
• Minimum of 2 years Med/Surg experience.

• ADN required, but BSN strongly preferred.
• Acute hospital experience preferred.

Responsibilities

• Collaborates to enhance the delivery of patient care services along the continuum of care.
• Evaluates, predicts, and facilitates the trajectory of patient care.
• Employs a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
