Job summary

Mass General Brigham seeks a Registered Nurse Case Manager to join our Case Management team at 55 Fruit Street in Boston. The role coordinates care across the continuum, driving efficient patient-focused outcomes and advocating for patients and families.

You must be a Massachusetts-licensed RN with at least 2 years of Med/Surg experience. BSN preferred; on-site, 40 hours per week with day shifts and weekend commitment.

Qualifications

• RN Case Manager experience required.
• Must be a licensed RN in the state of Massachusetts.
• Minimum of 2 years Med/Surg experience required.
• Case Manager certificate preferred.
• ADN required, but BSN strongly preferred.
• Acute hospital experience preferred.

• Collaborate to enhance patient care delivery along the continuum of care.
• Meet patient needs efficiently and expeditiously by improving the patient experience.
• Apply a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy and evaluation.
