Job summary

Mass General Brigham in Boston is hiring a Registered Nurse Case Manager to coordinate patient care and improve outcomes within the Case Management department at Massachusetts General Hospital. You will collaborate with clinicians to plan, implement, and evaluate care trajectories.

Applicants must hold an MA-licensed RN with at least 2 years Med/Surg experience; ADN required, BSN preferred; Case Manager certification preferred. Onsite, 40-hour day shifts at 55 Fruit Street.

Qualifications

• RN Case Manager experience required.
• Must be a licensed RN in the state of Massachusetts.
• Minimum of 2 years Med/Surg experience required.
• Case Manager certificate preferred.
• ADN required, but BSN strongly preferred.
• Acute hospital experience preferred.

Responsibilities

• Collaborates to enhance delivery of patient care along the continuum of care.
• Evaluates, predicts, and facilitates the trajectory of patient care.
• Uses assessment, planning, implementation, care coordination, monitoring, advocacy and evaluation.
