Job summary

Advocate Health’s Lexington Medical Center is seeking a Case Manager (Part‑Time, First Shift) to apply the nursing process to plan and evaluate care for defined patient populations. You will work with physicians, nurses, and allied staff to optimize outcomes and ensure continuity of care within a collaborative care team.

The role requires RN licensure, BSN preferred, and at least five years’ experience in caring for the identified population.

Qualifications

• Graduate of an accredited School of Registered Professional Nurses; BSN preferred, MSN encouraged.

Responsibilities

• Develop, implement, and evaluate case management outcomes for defined patient populations.
• Collaborate with patient care team to ensure coordination of healthcare services and continuity of care.
• Act as advocate and liaison to enhance communication and coordination across disciplines.
