Qualifications

• Minimum of five years of experience in the care of assigned patient population.
• Current registration to practice as a Registered Nurse in the applicable state.
• Case Management certification encouraged.

Responsibilities

• Develop, implement, and evaluate case management outcomes for defined patient populations.
• Collaborate with a patient care team including physicians, nurses, rehabilitative therapies, and other disciplines.
• Educate, consult, and assist in managing identified patient populations to achieve quality outcomes and cost-effectiveness.
• Facilitate timely achievement of patient outcomes through advocacy and coordination of care.
