Qualifications

• BSN required; RN license must be active.

Responsibilities

• Use RN process to guide care team toward optimal outcomes and resource utilization.
• Coordinate with patients, professionals, and community to ensure continuity of care.
• Conduct comprehensive needs assessments for identified patients and disease management goals.
• Develop and maintain accurate case records for referred patients.
• Document thoroughly using appropriate databases and documentation systems.
• Be knowledgeable about Medicaid, Medicare, and Managed Care reimbursement criteria.
• Demonstrate leadership in nursing practice and adherence to standards.
