Job summary

Advocate Health seeks an experienced RN in case management in Monroe, NC. This part-time role requires 32 hours per 6 weeks with shifts Mon-Fri 08:00–16:30 and weekends 07:00–19:00.

You will coordinate care, assess needs, and document plans of care across settings, ensuring compliance and optimal outcomes. You’ll manage records, navigate reimbursement criteria, and contribute to professional practice and continuous improvement within Advocate Health’s care teams.

Qualifications

• RN processes guide clinical team toward optimal outcomes and resource use.
• Document plans of care across settings and maintain accurate records.
• Knowledge of reimbursement criteria including Medicaid, Medicare and managed care.

Responsibilities

• Interacts with patients, professionals, and community to coordinate care and document plans.
• Develops and maintains case records for referred patients.
• Maintains documentation in correct databases and systems.
• Demonstrates nursing leadership and adherence to professional standards.
