Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, support discharge planning, and manage ongoing outpatient care under supervision of an RN or MD.

You will collaborate with Care Advocates and Social Workers, provide education and coaching, and participate in clinical development while delivering high-quality, compassionate care in a home or community setting.

Qualifications

• Current RN license (state(s) you are assigned to work)
• Valid driver's license, reliable transportation and liability insurance
• Current CPR certification
• 1+ years of clinical experience as a Registered Nurse (RN)

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by the patient's needs or documents rationale for not doing so
• Promotes patient health and independence through teaching and appropriate rehabilitative measures
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Regularly communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Participates in on-call responsibilities and services
• Participates in clinical development and continuing education programs
• Other duties as assigned
