Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum. You will assess needs, coordinate care, support discharge planning, and manage outpatient care.

Under supervision of an RN or MD, you will ensure appropriate level of care, provide education and treatment-decision support, and collaborate with a Care Advocate and Social Worker. Excellent teamwork is essential.

Qualifications

• Active RN license for state(s) of practice.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• At least 1 year of clinical RN experience.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
