Job summary

UnitedHealth Group in the United States seeks a Nurse Case Manager to guide patients and families through the care continuum, assess needs, coordinate care, support discharge and transition planning, and manage ongoing outpatient care under the supervision of an RN or MD.

You will educate, coach, and collaborate with a Care Advocate and Social Worker, performing on-call duties and contributing to clinical development and staff education while ensuring high-quality patient outcomes.

Qualifications

• Current RN license in the state(s) you are assigned.
• Current CPR certification.
• 1+ years of clinical RN experience.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and partners with the care team to plan a care approach.
• Makes referrals when indicated by patient needs and documents rationale if not done.
• Educates patients in self-care and rehabilitative activities to promote independence.
• Supervises LPNs and HHAs and completes documentation timely.
• Communicates patient progress to the clinical manager and care team, and plans staff education.
• Participates in on-call duties and ongoing clinical development.
