Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care.

Under supervision of an RN or MD, you will help ensure patients receive appropriate levels of care, provide education, coaching, and decision support, and collaborate with the Care Advocate and Social Worker.

Qualifications

• Current RN license valid in assigned state(s).
• Current CPR certification.
• 1+ year of clinical RN experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess patient needs and collaborate with the care team to develop and implement a plan of care.
• Promote patient health through teaching and appropriate self-care strategies.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Participate in on-call responsibilities and services.
• Collaborate with a Care Advocate and Social Worker on discharge planning.
