Job summary

MedStar Health is seeking a Registered Nurse to join the Case Management Team. You will apply clinical expertise to care coordination and discharge planning, ensuring high-quality, cost-effective, patient-centered care from pre-admission through post-discharge.

You will collaborate with multidisciplinary teams, develop integrated care plans, monitor outcomes, and ensure compliance with regulatory and reimbursement requirements across acute and post-hospital phases.

Qualifications

• ADN required; BSN preferred.
• 3–4 years clinical experience required; 1–2 years case management preferred.
• RN license in DC or MD depending on work location; CCM certification preferred.

Responsibilities

• Contributes to department goals while adhering to policies, procedures, and safety standards.
• Collaborates with multidisciplinary team to coordinate plan of care.
• Communicates with patient, family, and care team to align goals.
• Develops care plans across continuum with time-bound goals and necessary services.
• Evaluates patient response and updates plan with documentation and confidentiality.
