Healthier Mississippi People is seeking an RN-Case Manager to support patients under the care coordination framework. You will lead discharge planning from admission, coordinate with the care team, and ensure timely, safe transitions using the UMC nursing model.

Qualifications

• Must be a critical-thinker and able to prioritize patient clinical needs effectively for successful transition into post-acute care arena.
• Must be able to demonstrate the ability to take initiative, work interdependently, problem-solve, and meet goals and timeframes.
• Advocating for the patient, payer and the healthcare organization, creating healthcare continuity and quality patient outcomes.
• Behavioral and ethical standards that support and demonstrate actions in accordance with the UMC nursing model and state Board of Nursing.
• Customer service excellence recognizing and responding to patients, families, payers, staff, physicians and resources; timely referrals and liaison with departments.

Responsibilities

• Coordinates/ a multi-professional plan of care addressing discharge needs and anticipates clinical needs.
• Comprehensive assessment of clinical discharge needs using nursing processes.
• Coordination of care and services, case findings, follow-up assessments, and evaluation related to discharge planning.
• Rounds with attending/resident staff to identify patient discharge plans and provide clinically based recommendations.
• Maintains communications with care team and documents medical records to ensure continuity of care.
• Manages access to tests, procedures, and diagnostic results to reduce length-of-stay.
• Solves daily issues using nursing knowledge to ease patient transitions and presents cases to supervisors.
• Participates in employee council, researches best practices, and supports safe outcomes and financial sustainability.
• Identifies social determinants of health and works with social workers to mitigate readmission risk.
• Monitors department metrics and supports policy/practice changes through teamwork and innovation.
• Arranges services to reach outcomes within timeframes while maintaining holistic nursing focus.
