UMass Memorial Health is seeking a registered nurse with discharge planning and case management experience for a full-time role in Marlborough, MA. You will assess patient needs, design care plans, coordinate discharges and care transitions, and work with physicians and payers to optimize outcomes.

Required: MA RN license and 3 years in discharge planning/case management; strong organizational and communication skills; utilization management knowledge preferred.

Formación

• Three years clinical experience in discharge planning or case management.
• Ability to use specialized computer-based systems for clinical data gathering and reporting.
• Excellent organizational and communication skills.

Responsabilidades

• Assess patient needs and develop plans of care with appropriate resources.
• Coordinate discharge planning and transitions of care.
• Collaborate with physicians, care teams and payers on care planning.
• Educate practitioners on level of care and documentation requirements.
• Identify opportunities for improvement and participate in QA activities.
• Ensure compliance with regulatory requirements and standards.
