Job summary

Essentia Health seeks a registered nurse for Case Management/Social Services at St. Josephs Medical Center.

You will organize and provide patient care through assessment, planning, intervention, and evaluation, supporting the physician and care team to assure progress toward discharge and positive outcomes. The role integrates utilization management, care facilitation, and discharge planning for a designated caseload, with a daytime shift and 0.8 FTE.

Qualifications

• BSN or ADN degree from an accredited school or college of nursing
• Current RN licensure in appropriate state(s) of employment
• Basic Cardiac Life Support (BCLS) certification within 1 month
• Fargo: Basic Cardiac Life Support (BCLS) not required
