Job summary

Essentia Health is seeking a Registered Nurse for the Case Management - Social Services at St. Joseph's Medical Center. You will organize and provide patient care throughout assessment, planning, intervention and evaluation, while supporting the physician and care team to ensure progress toward discharge.

Responsibilities include coordinating utilization management, discharge planning, and care facilitation to maintain quality outcomes and efficient resource use for a defined caseload.

Qualifications

• RN licensure valid in appropriate state(s) of employment.
• BCLS certification within 1 month (with location-specific notes in placement)

Responsibilities

• Organize and provide patient care through assessment, planning, intervention and evaluation.
• Support physicians and the interdisciplinary team to ensure patient progress toward discharge.
• Coordinate utilization management, care facilitation and discharge planning for designated caseload.
• Plan effectively to meet patient needs, manage length of stay and optimize resource use.
