Job summary

Saint Francis Healthcare System is seeking a Part-Time Case Manager RN. The role requires coordination, discharge planning, and utilization review to improve outcomes while balancing cost. The nurse will apply case management practices appropriate to diverse patient ages and settings.

Qualified candidates hold a BSN (preferred), Missouri RN license, BLS, and at least 3 years of clinical nursing experience. Responsibilities include using the case management process to lead care delivery,

Qualifications

• Bachelor of Science in Nursing degree preferred.
• Currently licensed to practice as a Registered Nurse in the State of Missouri.
• Three (3) years clinical nursing experience.
• BLS Certification.

Responsibilities

• Lead case management activities to improve quality and outcomes.
• Coordinate discharge planning and resource allocation for care delivery.
• Apply case management process to monitor patient status and length of stay.
• Collaborate with interdisciplinary team to balance quality and cost in care.
