Job summary

HCA HealthONE Aurora is seeking a Case Manager RN to guide patients and families through every step of their care journey and coordinate services within a multidisciplinary team.

You will develop individualized treatment plans, complete psychosocial assessments, document findings, and collaborate with physicians, nurses, and department leaders to ensure safe, smooth transitions and high-quality care.

Qualifications

• RN license required.
• 2 years of experience in Case Management OR 3 years experience in clinical nursing required.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy and insurance benefits.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths, needs, and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare values of compassion, respect, and excellence in every interaction.
