Job summary

HCA HealthONE Swedish Medical Center is seeking a Case Management Coordinator to oversee interdisciplinary coordination of clinical, financial, and psychosocial care. You’ll balance patient needs with efficient resource use, initiating discharge planning early and guiding care throughout hospitalization.

The role requires Colorado licensure as RN or LCSW, BLS certification, and 5+ years of clinical UM/CM experience, with supervisory experience preferred.

Qualifications

• RN or MSW licensure/credentials required.
• Graduate of an accredited school of nursing or social work.

Responsibilities

• Coordinate discharge planning with discharge planners, social workers, physicians.
• Assess admission appropriateness and level of care during hospitalization.
• Monitor quality and risk management and report issues to leadership.
• Provide leadership and supervision in UM/CM/department activities and staff development.
