Job summary

MyMichigan Health is seeking an RN Case Manager to coordinate patient care across the continuum, from admission through discharge, ensuring timely and appropriate levels of care. You will collaborate with physicians and the care team to ensure medical necessity and optimal transitions.

Responsibilities include care coordination rounds, discharge planning, and interfacing with payers to negotiate patient status while maintaining high quality and compliant care delivery.

Qualifications

• Three to five years clinical experience.
• Certification in case management preferred.
• Recommend ACM certification after two years of experience.

Responsibilities

• (30%) Work with healthcare team, patient, family to transition the patient to appropriate level of care.
• (20%) Collaborates with Care Management Team to provide admission, discharge criteria, and regulatory standards.
• (20%) Lead daily care coordination meetings and document next steps and available days.
• (10%) Facilitate discharge planning including extended care placements and home care.
• (10%) Determine necessity for second-level review and negotiate status with payer.
• (10%) Analyze admission, concurrent, and retrospective data to assure quality of care.
