Qualifications

• Bachelor's degree in Nursing; or an ADN/ Diploma with a Master's in Nursing.
• Current State of Michigan Board of Nursing licensure.
• Minimum of 3 years of acute care RN experience or 3 years in care management.
• At least 1 year in nursing management or case management progression.

Responsibilities

• Assess and develop a comprehensive plan of care for patients in inpatient and ED.
• Lead daily care coordination rounds to update the plan and ensure implementation.
• Initiate discharge planning and referrals to homecare, DME, and infusion services.
• Collaborate with social workers, case managers, and care navigators to optimize transitions.
• Perform clinical reviews on admission and monitor every 3 days or as required by payers.
• Identify opportunities for cost reduction and participate in utilization management initiatives.
