Job summary

McLaren Health Care in Michigan is seeking a registered nurse to join the Case Management team as a patient care coordinator. You will assess patient needs, coordinate discharge planning, and collaborate with physicians and social services to ensure timely transitions.

Daily work times are 8:00 am to 4:30 pm, shift Days, full-time with bi-weekly hours of 80. Requires RN license, BSN, and 3 years of acute hospital experience; BLS is preferred.

Qualifications

• RN licensure is required and active.
• Bachelor's degree in nursing or higher is required.
• Minimum of 3 years acute hospital care experience.

Responsibilities

• Perform care coordination assessments for initial admissions within 24 hours and for readmission/transition planning.
• Collaborate with social workers, physicians, and other disciplines to ensure safe, timely transitions to appropriate care levels.
• Assess patient/family needs to reduce barriers and formulate discharge plans.
• Identify unsigned LOC orders and coordinate with utilization management and providers.
• Review DRG/LOS data to plan discharge needs and identify a primary contact.
• Assess readmission risk and initiate interventions to improve transitions of care.
• Coordinate discharge planning and referrals to social services, post-acute placement, and other agencies per SOP.
