Job summary

Midland Memorial Hospital is seeking a Registered Nurse Case Manager to lead individualized care plans. You will collaborate with physicians, social workers, and the care team to assess needs, coordinate services, and pursue timely discharge planning and post-acute care arrangements.

Ideal candidates hold Texas RN licensure, at least five years in acute care or case management, and CCM certification is preferred.

Qualifications

• Current State of Texas license as a Registered Nurse.
• Five years of acute care or case management experience.
• Ability to apply nursing theory, evidence-based nursing knowledge, problem solving skills and therapeutic techniques to perform professional nursing duties.
• Certified Case Manager (CCM) preferred.

Responsibilities

• Establish case management plans with goals to meet patient needs.
• Coordinate with care team to plan, implement, monitor and evaluate services.
• Participate in face-to-face meetings with patients, families, and providers.
• Refer complex psychosocial issues to Social Worker when needed.
• Maintain accurate documentation and ensure timely discharge planning.
