Job summary

Independent Health is seeking an RN Case Manager who will apply the six essential activities of case management, including assessment, planning, implementation, coordination, monitoring, and evaluation. The role requires CCM certification and an RN license in an NLC state, with relocation or travel alignment as needed.

Responsibilities include evaluating patient needs, coordinating care across disciplines, and ensuring compliant, cost-effective service delivery.

Qualifications

• RN license in an active, current, unrestricted state (NLC).
• CCM certification required; if not held, must obtain within 3 years of start.
• 3 years of clinical RN experience in med/surg or ambulatory care.
• Experience in managed care is a plus.
• Clinical knowledge of health or social work needs for the population served.

Responsibilities

• Assess patients' immediate and long-term needs and develop a care plan.
• Coordinate and monitor care with the interdisciplinary team to ensure quality outcomes.
• Ensure compliance with regulatory and privacy standards and proper documentation.
• Advocate for patients and identify barriers to effective care management.
• Collaborate with payors, providers and team members to evaluate cost-effectiveness.
