Job summary

Independent Health Association in Buffalo, NY is seeking a licensed RN to join our Case Management team to apply the full scope of case management, including assessment, planning and coordination of care.

You will collaborate with physicians and the care team to ensure regulatory compliance, effective communication, and optimal patient recovery. CCM certification must be obtained within three years of starting, with a competitive hourly wage and comprehensive benefits.

Qualifications

• RN with active, unrestricted license in NLC state.
• CCM certification required; must obtain within 3 years if not held.
• Minimum 3 years of clinical RN experience in med/surg or ambulatory care.
• Experience in managed care a plus; strong communication and advocacy skills.

Responsibilities

• Assess patient needs through social and medical history.
• Develop care plans considering physical and psychosocial needs and cost factors.
• Evaluate and monitor the plan within case management scope.
• Ensure regulatory compliance, privacy, and documentation accuracy.
• Build relationships with the inter-disciplinary team and communicate objectives.
• Advocate for patients and research community resources.
