Required BSN or higher, active RN license, 1+ year case management experience, and strong communication. EHR proficiency is expected.

Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered Registered Nurse license.
• 1+ years experience in case management.
• Strong interpersonal and communication skills (written and verbal).
• Proficiency in electronic health records (EHR) software.

Responsibilities

• Conduct comprehensive assessments of patients and psychosocial needs.
• Collaborate with healthcare teams to develop individualized care plans.
• Monitor progress and adjust care plans as needed.
• Serve as the primary liaison between patients and providers for care coordination.
• Maintain up-to-date records and ensure confidentiality and compliance.
