Qualifications

• Minimum three (3) years of acute care clinical experience.
• Two (2) years’ experience clinical Case Management or Utilization Review preferred.
• Completion of an accredited Registered Nursing program.

Responsibilities

• Case Managers are responsible for identifing and prioritizing the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting, desire for care and responses to any previous care.
• Initiates discharge planning for each patient upon admission as appropriate.
• Demonstrates sound judgement and effective decision making in following the nursing process.
• Case Managers may float to the other facility within Southwest Health Rancho Springs and Inland Valley Hospitals as needed.
