Job summary

Southwest Healthcare Inland Valley Hospital and Rancho Springs Hospital are part of Universal Health Services (UHS). They seek a Registered Nurse with case management experience to assess patient needs, initiate discharge planning, and coordinate care across two hospitals in California.

The role requires an active CA RN license and may involve floating between facilities as needed. The incumbent will apply clinical judgment within regulatory and hospital policies, participate in utilization

Qualifications

• Minimum three (3) years of acute care clinical experience.
• Two (2) years’ experience clinical Case Management or Utilization Review preferred
• Completion of an accredited Registered Nursing program.
• Current California Registered Nurse license
• CCM and/or CPUR certification preferred

Responsibilities

• Case Managers are responsible for identifing and prioritizing the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting, desire for care and responses to any previous care.
• Initiates discharge planning for each patient upon admission as appropriate.
• Demonstrates sound judgment and effective decision making in following the nursing process.
• Employees may be required to float to the other facility within Southwest Health Rancho Springs and Inland Valley Hospitals based on operational needs.
