Job summary

Universal Health Services (UHS) is seeking a dedicated Case Manager to support Inland Valley and Rancho Springs Hospitals in Wildomar and Murrieta, California. The role centers on identifying needs, coordinating assessments, and initiating discharge planning with adherence to policies and core values.

The ideal candidate has 3+ years of acute care clinical experience, plus 2+ years in Case Management or Utilization Review, and has completed an accredited RN program.

Qualifications

• Minimum three (3) years of acute care clinical experience.
• Two (2) years’ experience in Case Management or Utilization Review preferred.
• Completion of an accredited Registered Nursing program.

Responsibilities

• Case Managers identify and prioritize the need for further assessment based on the patient’s presentation and care setting.
• Initiates discharge planning for each patient upon admission as appropriate.
• Demonstrates sound judgment and adheres to hospital policies and core values.
