Job summary

Stoughton Health is seeking a qualified RN Case Manager to join our Medical‑Surgical team in a part-time role. This in-person position at Stoughton Hospital supports discharge planning and care coordination across departments and external agencies.

The successful candidate will have an Associate’s Degree in Nursing (Bachelor’s preferred), five years of acute-care experience, and an active Wisconsin RN license. Proficiency with Microsoft Office and healthcare communications is expected.

Qualifications

• Five years acute‑care nursing experience.
• Prior related work in Patient Review or Case Management is a plus.
• RN licensure in Wisconsin in good standing.

Education

Preferred: Bachelor’s Degree.

Experience
Required: Five years acute‑care nursing experience.
Preferred: Prior related work experience in Patient Review or Case Management.

Licensure
Registered Nurse licensed in good standing by the Board of Licensing and Regulation in the State of Wisconsin.

Computer Competency
Basic computer skills. Microsoft Office Suite: opening a document, changing font, saving, and writing a letter.

Work Shift

Part‑time, 0.5 FTE. Week 1: 24 hours. Week 2: 16 hours. Variable schedule Monday‑Friday, 8:00 a.m. to 4:30 p.m.; no weekends.
