Stoughton Health is seeking a qualified RN Case Manager to join our Medical‑Surgical team in a part-time role. This in-person position at Stoughton Hospital supports discharge planning and care coordination across departments and external agencies.

The successful candidate will have an Associate’s Degree in Nursing (Bachelor’s preferred), five years of acute-care experience, and an active Wisconsin RN license. Proficiency with Microsoft Office and healthcare communications is expected.

Qualifications

• Five years acute‑care nursing experience.
• Prior related work in Patient Review or Case Management is a plus.
• RN licensure in Wisconsin in good standing.

Responsibilities

• Review benefits for planned admissions.
• Conduct concurrent chart review for medical acuity.
• Coordinate admissions to supportive care.
• Participate in discharge planning within the multidisciplinary care team.
• Assist in assignment of DRGs.
• Communicate with hospital departments and outside agencies.
